Week 1 Summary (March 24-28, 2014).

Six clinics (two diabetes clinics, one diabetes in pregnancy clinic, one diabetic foot clinic, two general endocrinology clinics) this week with total of 243 patients seen by our team (Helen Yifter Bitew, Abdurazek Ahmed Abdela, 4 Medicine Residents, Katy Brown, Mike McDermott). Among these were some severe Graves' disease cases, one severe Cushing's syndrome case in an 18 year old woman, congenital hypothyroidism and one case of a severe deforming metabolic bone disease in a 14 year old boy.

Two inpatient rounding sessions (one for Endocrinology and one for General Medicine). 22 patients were seen: many diabetic patients, several severe foot ulcers, one 80 year old woman with thyroid storm (storm score of 70), one diabetic patient in a prolonged coma from severe prolonged hypoglycemia, and several cases of leukemia and disseminated TB, and one case each of lung cancer, lymphoma, bacterial endocarditis, juvenile rheumatoid arthritis, hemophilia and one case of severe obesity with pulmonary hypertension in a 20 year old man.

Six education sessions (PowerPoint presentations) to fellows and residents by Mike McDermott.

I hope to see Lucy this weekend.
Cell phone stolen by a pickpocket today. No pictures today. Maybe again next week.
Friday Afternoon Endocrine Clinic. Saw 20 patients. The disorders these patients had included multiple cases of hyperthroidism (both Graves' disease and toxic multinodular goiter) with one patient being in the second trimester of pregnancy. All patients were treated with propylthiouracil (PTU) because neither Carbimazole nor Methimazole are available in Ethiopia. One 18 year old woman had severe, newly diagnosed (today) Cushing's syndrome.
Friday Morning inpatient Medicine Rounds. One day each week the two Endocrine fellows, Abdurazek Ahmed Abdela and Helen Yifter Bitew, serve as attendings for the General Medicine Inpatient Service. Katy Brown and Mike McDermott attended on these rounds today also. We saw 16 inpatients that were presented to us by the Medicine Interns and Residents. Among the cases that we saw were 3 with acute leukemia, 1with CLL and a blast crisis, 1with a lymphoma, one with hemophilia and a hemarthrosis, 1 with newly diagnosed lung cancer and a pleural effusion, 3 with disseminated TB, 1 with severe Juvenile Rheumatoid Arthritis, 1 with bacterial endocarditis, 1 (age 20 years old) with severe obesity and pulmonary hypertension and 1 type 1 diabetic in a coma from brain damage sustained during a severe hypoglycemic episode.
